Intensive Outpatient Program Registration

Program Compliance Agreement: Before acceptance into the program, you will be expected to sign a contract agreement to follow all the program rules. These will be fully explained to you. However, always ask questions if you do not understand any information. Your success depends on you fully understanding all program guidelines.

Fees: You will have to pay a one-time orientation fee of $20 plus $80.00 for the total cost of the classes and a $40 workbook fee. Fees must be paid during the orientation phase. Participants will not be able start the program until all fees are paid up front. (money orders only no exceptions).

Programs /Supervision: You will be given a plan of treatment during the orientation phase. This plan will also include program attendance requirements. You are expected to attend all required meetings and be on time. Any absence from treatment group must be approved by the treatment counselor and or treatment supervisor prior to the missed class. Medical documentation must be provided if absence is due to sickness/illness or death in the immediate family no exceptions. If a participant received three unexcused absences, you will be terminated from the program.

Use of Drugs and Alcohol: You are not allowed to use illegal drugs, substances, or alcohol while on the program. All participants must be drug and alcohol free.

Medications: Before taking any prescribed or over-the- counter medications, you must first notify your treatment counselor. All prescriptions for medications must be verifiable. You must provide a letter from physician regarding medications that are prescribed prior to starting treatment.

Urine Drug Screens (UDS): Random drug and alcohol testing is part of the program requirements. You will be asked to submit to a drug and alcohol test at least three times while in treatment. These random tests can occur Monday thru Friday.

If a participant tests positive (which includes refusals or rejected samples), the following things will occur:

Medical Emergencies: All medical emergencies must be reported as soon as possible.

Check In: Each time you report you must sign in and sign out when you leave. Failure to sign in and out will result in the day being counted as absence.

Proper Attire: Appropriate attire, including shirt, shoes and bra is required. No gang related attire, shirts and language on it deemed offensive to staff, "wife beaters" sagging pants, halter-tops, or mini dresses cannot be worn in the building.

Reinstatement of Clients: If you are terminated from IOP, you will have to be reinstated back into IOP with 100.00 m/o and sessions will start over from the beginning (any previous positive drug screens will stay).

You Have the Right:

1. To considerate and respectful care;
2. To reasonably expect, from the staff members responsible for your care and welfare, complete and current information of your condition, diagnosis, treatment, and prognosis in terms and language you can understand unless it is medically inadvisable to give you that information and so documented in your medical records;
3. To know by name and specialty, if any, the staff members responsible for your care; to know some counselors and/interns may be working under clinical supervision in an effort to obtain their license.
4. To consideration of your individuality, and to be treated with consideration, respect, and full recognition of your dignity;
5. To respectfulness and privacy as it relates to your therapy program. Case discussions, consultations, examinations, and treatment are confidential and should be conducted discreetly;
6. To expect DUI Safety Center to make a reasonable response to your requests;
7. To expect reasonable continuity of care, which includes schedules of services;
8. To be fully informed, prior to or at the time of admission and during your stay; of the services available at DUI Safety Center Treatment Program and of related changes;
9. To be discharged for therapeutic reasons, for your welfare, or that of other participants;
10. To be encouraged and assisted, throughout your period at DUI Safety Center, to understand and exercise your rights as a participant and to this end, you may voice your grievances and recommend changes;
11. To be free from mental and physical abuse, and free from chemical and physical restraints except in emergencies, or as authorized in writing by physician for specified and limited periods of time and when necessary to protect you from injury to yourself and others;
12. To be assured confidential treatment of your personal and medical records;
13. To be fully informed as evidenced by your written acknowledgment, prior to or at the time of admission and during your treatment at DUI Safety Center, of the rights and responsibilities set forth herein and of all the rules and regulations governing participant conduct and responsibilities.
14. To be accorded freedom for the use of bathrooms when needed.

You Have the Responsibility:

1. To remain totally abstinent of all mood-altering chemicals.
2. To be honest about matters that relate to you as a participant.
3. To attempt to understand your problem.
4. To attempt to follow the directives and advice offered by the staff.
5. To know the staff members who are caring for you.
6. To report changes in your condition to those responsible for your care and welfare.
7. To be considerate and respectful of the rights of both the fellow participants and staff.
8. To honor the confidentiality and privacy of other participants.
9. To use the grievance procedure if you feel your rights are being violated.
10. To keep appointments and cooperate with the staff.
11. To avoid making unreasonable demands.
12. To take an active part in your therapy program; this includes going to all groups, lectures, and other group activities.
13. To comply with the policies and expectations of DUI Safety Center.
14. To take an active part in group therapy sessions by discussing your problems as they relate to your addictive, violent and/or criminal behavior.
15. To obey all the rules of the DUI Safety Center while you are in the treatment program.

Confidentiality Of Alcohol And Drug Abuse Patient Records

The confidentiality of alcohol and drug abuse patient records maintained by this program is protected by Federal law and regulations. Generally, the program may not say to a person outside the program that a patient attends the program, or disclose any information identifying a patient as an alcohol or drug abuser.

Unless:

1. The patient consents in writing, or
2. The disclosure is allowed by a court order, or
3. The disclosure is made to medical personnel in a medical emergency or to qualified personnel for research, audit, or program evaluation.

Violation of the Federal law and regulations by a program is a crime. Suspected violations may be reported to appropriate authorities in accordance with Federal regulations.

Federal law and regulations do not protect any information about a crime committed by a client either at the program or against any person who works for the program or about any threat to commit such a crime.
