Program Compliance Agreement: Before acceptance into the program, you will be expected to sign a contract agreement to follow all the program rules. These will be fully explained to you. However, always ask questions if you do not understand any information. Your success depends on you fully understanding all program guidelines.
Fees: You will have to pay a one-time orientation fee of $20 plus $80.00 for the total cost of the classes and a $40 workbook fee. Fees must be paid during the orientation phase. Participants will not be able start the program until all fees are paid up front. (money orders only no exceptions).
Programs /Supervision: You will be given a plan of treatment during the orientation phase. This plan will also include program attendance requirements. You are expected to attend all required meetings and be on time. Any absence from treatment group must be approved by the treatment counselor and or treatment supervisor prior to the missed class. Medical documentation must be provided if absence is due to sickness/illness or death in the immediate family no exceptions. If a participant received three unexcused absences, you will be terminated from the program.
Use of Drugs and Alcohol: You are not allowed to use illegal drugs, substances, or alcohol while on the program. All participants must be drug and alcohol free.
Medications: Before taking any prescribed or over-the- counter medications, you must first notify your treatment counselor. All prescriptions for medications must be verifiable. You must provide a letter from physician regarding medications that are prescribed prior to starting treatment.
Urine Drug Screens (UDS): Random drug and alcohol testing is part of the program requirements. You will be asked to submit to a drug and alcohol test at least three times while in treatment. These random tests can occur Monday thru Friday.
If a participant tests positive (which includes refusals or rejected samples), the following things will occur:
Medical Emergencies: All medical emergencies must be reported as soon as possible.
Check In: Each time you report you must sign in and sign out when you leave. Failure
to sign in and out will result in the day being counted as absence.
Proper Attire: Appropriate attire, including shirt, shoes and bra is required. No gang
related attire, shirts and language on it deemed offensive to staff, "wife beaters" sagging
pants, halter-tops, or mini dresses cannot be worn in the building.
Reinstatement of Clients: If you are terminated from IOP, you will have to be reinstated back into IOP with 100.00 m/o and sessions will start over from the beginning (any previous positive drug screens will stay).
